PROJECT S.O.A.R. |

Project S.O.A.R. is an acronym for Stability, Opportunity, Achievement and Recovery and provides transitional supportive housing for 70 homeless men and women (55 beds for men, 25 beds for women). Located on Long Island, this transitional housing program primarily serves guests who have jobs in the community or are enrolled in one of the shelter's job training or educational programs The goal of Project S.O.A.R. is to facilitate the program participant's transition out of the shelter and into permanent housing.

Stability: Residents are provided a safe, comfortable, alcohol and drug-free, supportive living environment.

Opportunity: Assistance is given to the areas of life skills development, substance abuse counseling, case management, education, employment, housing search, community re-integration and aftercare support.

Achievement: The goal of the program is to help participants achieve permanent housing.

Recovery: Support is provided to participants in all areas of their lives to ensure a successful recovery.
Project S.O.A.R. provides comprehensive services including:
- Case management
- Mental and medical health assessment and referral
- Substance abuse counseling
- Legal advocacy
- Life skills training and referrals to job training and education
- Housing search and placement assistance
Eligibility:
- Homeless
- 18 years or older
- Employed, collecting benefits or enrolled in or awaiting enrollment in an educational or job training program.
- Posses a desire to transition into permanent housing
- Be able to participate in and contribute to communal living
- Demonstrate commitment to ongoing personal recovery
- A minimum of 30 days of sobriety

SAFE HARBOR |
| The Safe Harbor Program serves 20 homeless men and women who are HIV positive and recovering from substance abuse. Safe Harbor offers a variety of services designed to assist the client in their journey towards independent lives in the community.
Client Services:

Individual counseling and education for HIV and substance abuse

On-site medical and mental health care

Medication management support

Case management and client advocacy

Aftercare support and planning
HIV Through daily community meetings, HIV education groups and individual teaching, clients discover new ways of handling the stress of living with HIV. Clients are assisted in accessing health care and other community resources and services.
Recovery Safe Harbor’s program is focused on recovery and stabilization. A holistic approach to treatment is developed and is based on individual needs. Case managers, nurses, substance abuse counselors and social workers are available to help clients develop healthier lifestyles.
Housing After completing the program, clients are placed in more stable housing and are followed by and aftercare specialist. Ongoing support services are offered to individuals in the community.

- Homeless
- HIV+, with physician documentation
- 18 years of age or older
- Able to perform activities of daily living independently
- Willing to address substance abuse
- Willing to participate in and contribute to educational/support groups
