Transitions
Transitions is a short-term residential program for homeless men and women in early substance abuse recovery. The program operates 70 beds for approximately 20 days and is open to those clients who have completed the detoxification process and seek placement in longer term residential therapeutic programs, outpatient programs, or other substance abuse treatment programs.
Transitions offers clinical and support services including the following elements:
- Case management and individual service plans
- Individual and group counseling
- Stress management
- Substance abuse education and relapse prevention
- HIV-AIDS education
- Tobacco and gambling cessation counseling
- Family unification
- Aftercare planning and referrals to halfway houses or residential programs.
The goal of Transitions is recovery. Transitions provides a safe, therapeutic environment and services which help prepare the client physically, mentally and socially for an alcohol and drug free lifestyle.
This recovery program focuses on relapse prevention, behavior modification, interpersonal skill development, and re-socialization skills. Peer support groups meet daily to explore life issues related to substance abuse. Didactic education sessions provide clients information about physiological, psychological and sociological consequences of substance abuse. One to one goal-oriented sessions assist clients in choosing an appropriate placement for continuation of treatment.
Eligibility:
- Men and Women 18 years or older
- Clients physically and mentally stable and not at risk of withdrawal
- Must be willing to be active participants, focusing on recovery and the identification of long-term goals
