 SUBSTANCE ABUSE PROGRAMS/TRANSITIONS


Transitions
The Boston Public Health Commission operates Transitions, an evidence-based, short-term treatment program for adults. Transitions has 40 beds and is located on the Long Island Health Campus in the Boston Harbor.  

The goal of Transitions is recovery and to offer a safe and therapeutic environment. Transitions focuses on relapse prevention, behavior modification, interpersonal skill development, and re-socialization skills. 

Peer support groups meet daily to explore life issues related to substance abuse. Educational sessions provide clients information about physiological, psychological, and sociological consequences of substance abuse. One-to-one, goal-oriented sessions help clients to choose a suitable placement for continuation of treatment.

Services provided:

  • Case management and individual service plans
  • Individual and group counseling
  • Stress management
  • Substance abuse education and relapse prevention
  • HIV-AIDS education
  • Tobacco and gambling cessation counseling
  • Help with family unification
  • Aftercare planning and referrals

Eligibility:

  • 18 years or older
  • Clients physically and mentally stable and not at risk of withdrawal
  • Willing to address substance abuse and identify long-term goals
  • Participation in the educational and/or support groups offered

Edwin Willis has been with the Commission for thirteen years and is a Substance Abuse Coordinator for the Homeless Services Transitions Program. As a Substance Abuse Coordinator, Edwin is responsible for overseeing the provision of case management and substance abuse services to the clients of the Transitions Program. Edwin is a mentor to staff and a great advocate for clients of the program. Sharing his knowledge and skills is something that he strives to do and is welcomed by everyone he works with. He always shows initiative and is a great problem solver which is an asset to this program. Edwin has fostered partnerships with many halfway houses, hospitals and other substance abuse programs all in an effort to ensure that clients can be properly referred and receive the treatment they deserve.
