Below is a list of resources and tools developed specifically for and by the Center and our partners grantees. These tools can be used to guide short and long term institutional and organizational planning and programming efforts for health equity.

Mayor's Task Force Blueprint Report: Mayor Menino’s blue ribbon task force, comprised of leaders from academic institutions, community coalitions, health care, and insurance providers, developed a blueprint, with strategies that address social determinants and health factors that contribute to health disparities. more>>

Hospital Working Group Report: Mayor Menino’s Hospital Working Group, comprised of representatives from Boston’s hospitals and community health centers, developed hospital-based recommendations around the collection and reporting of disparities data, coordination of institutional cultural competency efforts, diversification of the health care workforce, and strengthening of community linkages.

Disparities Data Report: The Commission’s data report takes a comprehensive look at health disparities, including the social determinants of health such as racism, the burden of poverty, and the declining availability of affordable health insurance.

Year One Report: Formative evaluation of Year One activities to address racial and ethnic health disparities as identified by the Hospital Working Group and the Mayor’s Task Force. This report includes a review of successes, lessons learned, and next steps.

The Jamaica Plain Youth Health Equity Collaborative, a group of youth, health care, housing, education, and youth serving organizations, met for over a year to look at six social determinant areas or BUCKETS that impact young people who live, work, and play in Jamaica Plain. We hope that in the hands of young people, their families, and the organizations that serve them, this report will aid in the fight to create a more equitable JP and Boston. |
CCHERS Sophie Davis Plan: This plan was developed by the Center for Community Health Education Research and Service (CCHERS). It outlines steps on how to provide pathways that bring more minority and/or socio-economically disadvantaged students into the medical profession.
Cultural Competency Assessment Tool: Designed in three sections, the Cultural Competence Assessment Tool offers health care organizations a framework for assessing the delivery of culturally and linguistically appropriate services.
Data Collection Toolkits: Model toolkits for hospitals, health centers, and community organizations to collect information by race, ethnicity, preferred language, and highest level of education. There are two recommended toolkit templates. Template A collects information for Latino/Hispanic individuals within the race category and Template B collects Latino/Hispanic as a separate question. If you are collecting information on Latino/Hispanic as a racial category refer to Template A. If you are collecting information on Latino/Hispanic identity as a separate question use Template B.
Youth Bridge Programs Review: A comprehensive literature review that can be used by youth health career programs and schools to assist in planning and programming. The review highlights existing types of youth-to-health careers pipeline programs and best practices along with key elements for success.
Unnatural Causes Youth Companion Guide: The Youth Companion Guide designed to accompany the Unnatural Causes film series. The Guide can be used in both after school and in-school time settings.
