ABOUT THE CHRONIC DISEASE PREVENTION AND CONTROL DIVISION |
|
The Chronic Disease Prevention and Control Division takes a lead role in the Commission's initiatives to reduce the overall burden of chronic disease and address disparities in outcomes, treatment, and related services. We focus on improving the public health system across the continuum of: preventing, screening and early detection; and controlling and treating chronic disease.
Key areas that we focus on include:
1. Promoting policy, environmental, and systems changes that build healthier communities, support healthy behaviors, and improve health outcomes;
2. Increasing public awareness about the benefits of and resources for healthier lifestyles;
3. Providing direct services, resources, and education for high-risk populations.
4. Supporting implementation of evidence-based practices
5. Improving access to and coordination of treatment and community supports
6. Providing leadership and building partnerships for a coordinated citywide approach
