Premium Assistance Programs

MassHealth Family Assistance Programs
There are two different programs under MassHealth Family Assistance so read them carefully. You can also call one of the resources listed if you have questions or want more detail.

Premium Assistance Program
This program is for families with children under age 19 who have health insurance at a job and whose income is between 150 and 200 percent of the poverty level. (See income chart. )

What are the benefits of Premium Assistance?
Premium Assistance will help pay for some of the cost of your or your spouse's employer's health plan. Your child's benefits are the same as the employer's plan.

How much will it cost?
Your share of the cost for each child, under age 19 is $10 per month (family maximum of $30 per month). MassHealth will pay the rest.

How do I apply?
Call or visit one of the MassHealth enrollment sites near you. You can also call (888) 665-9993 and get an application called a Medical Benefits Request (MBR) form mailed to you. You will need to provide proof of your family's income (pay stubs or tax returns).

Purchase of Medical Benefits
This program is for children under age 19 who have no insurance and are not covered under a parent or guardian health plan, and whose family income is between 150 and 200 percent of the poverty level (see income chart ).

What are the benefits of this program?
Family Assistance under MassHealth covers a wide range of services, including hospital and doctors' visits, dentists' visits, lab services, prescription drugs, medical equipment, medically necessary transportation, home nursing care and mental health and substance abuse services.

How much will it cost?
Your share of the cost for each child under the age of 19 is $10 per month (family maximum of $30 per month). MassHealth will pay the rest.
