Immunization Schedule

It's Never Too Late to Vaccinate!



To protect your child from serious childhood diseases, your child needs a basic series of vaccinations before
2 years of age and booster doses for some vaccines at 4-6 years of age and at 11-12 years of age.

 at 2 months  
DTaP/DTP, Polio, Hib
 at 4 months 
DTaP/DTP, Polio, Hib
  at 6 months
DTaP/DTP, Hib
  from 12 - 15 months
Hib, MMR (Measles-Mumps-Rubella)
  from 12 - 18 months
Polio, Varicella (Chicken pox)
  from 15 - 18 months
DTaP/DTP
  from Birth - 18 months 
Hepatitis B (3 doses)

  from 4 - 6 years 


DTaP/DTP, Polio, MMR

(Varicella if child did not have the disease chickenpox or the vaccine earlier)
Hepatitis B (if not received earlier)
  from 11 - 12 years
Td, (MMR, Varicella & Hepatitis B (if not received earlier)
 
The above are the routinely recommended ages. If your child has missed shots, your child can be caught-up and protected. Talk to your child's doctor if you have questions.
