INFECTIOUS DISEASE REPORTING - Laboratories

research and labs All clinical laboratories in the City of Boston are required to report specimens that have tested positive for certain communicable diseases to the Boston Public Health Commission (BPHC).  In addition, City of Boston regulations require certain research laboratories to report illness, exposure, or absenteeism among staff members. Hospital Laboratory Reporting Form pdf

  • This form should be used by hospital laboratories when reporting a positive communicable disease specimen.

  • A complete list of all communicable diseases that are reportable by hospital laboratories and be found on Page 2 of the reporting form.

  • Please fax the completed form to (617) 534-5905 or call (617) 534-5611.

Infectious Disease Research Laboratory Reporting Form pdf

  • This form should be used by research laboratories to report illness, significant exposure, or absenteeism meeting specified criteria among persons working with covered agents.   

  • This requirement applies only to infectious disease research labs that work with specified high risk agents.  For a list of covered agents, click here.
